E-Visa Application Form - Bermuda
You can get an electronic Visa for travelling Bermuda by submitting an online application.
The form requires your personal, passport, and visit details.

Bermuda Travel Authorisation (Mariners)
The Quarantine Authority (Minister of Health) of Bermuda, as per paragraph 6 of the Quarantine (COVID-19) (No.3) Order requires visitors to have a negative pre-departure test result to travel to Bermuda, taken no more than 5 days before departure. As of 11 July 2020 visitors without a negative pre-departure test result cannot obtain a Travel Authorisation. If you are at least 10 years old , please upload your negative pre-departure PCR COVID-19 test result document here. You will not be able to complete your Travel Authorisation without uploading your result. If you are aged 9 years or under, you are not required to have a pre-flight test so please upload proof of age instead. I have had a pre-departure test: (Yes I took a PCR COVID-19 test before arrival. I will upload the file here AND have the results either printed or on my phone or tablet to show the public health officer upon arrival in Bermuda. (Note: Children 9-years-old and younger do not require a pre-departure test, upload proof of age instead). )

Covid-19 Important Announcement
Bermudan Officials require you to take a pre-departure PCR Covid-19 Test within five days before the departure.
You need to submit the negative test results in the form.
In case you do not take the PCR test, you will face the denial of e-Visa.
If your age is above ten years, you need to upload the document showing negative COVID-19 Test results.
For a child(ren) of age nine years are under, they do not require a pre-departure COVID-19 Test, you have to upload the proof of their age.
There is a questionnaire related to symptoms of COVID-19 at the end of the application that evaluates your health status.
Instructions For Filling Out Application Form
If you plan to visit Bermuda, you can apply for a Resident, Visitor, or Mariner’s Travel Authorization.
Who Can Apply?
Those aged above 18 years will complete the application on their own.
A parent or legal guardian would complete the application for his child(ren) or dependent(s) under 18 years.
You may fill the following requirements:
Provide your intended date of arrival in Bermuda.
Parent/Legal Guardian Name and Email: Enter your name and email in this entry if this application form is for your child or dependent. Otherwise, you can leave this entry unfilled.
I had a Pre-Departure Test: Select “YES” if you (or your child aged at least ten) took the test. If you are filing the application for your child aged under 9, you can select “No, I am minor”.
File Upload: In this tab, upload the document of your pre-departure PCR Covid-19 negative test results or, if this visa application is for your child aged under 9, upload the document showing age-proof.

Address while in Bermuda: Select the address where you will stay in Bermuda, it can be guesthouse or bed and breakfast, hotel, boat or yacht, educational institute, friend or family house, private homes or rental house. If you can’t find your address on the list, you can write in the next tab.
If you have booked Bed and Breakfast then provide the name and contact details of your host.
If you will stay with a resident of Bermuda, in this case, provide the name, contact details, and the address of the resident.

Health Insurance: I confirm that I am travelling with health insurance that covers illness and injury outside of my home jurisdiction, including those related to a positive COVID-19 diagnosis while in Bermuda (and will have a printed or electronic copy to produce on arrival in Bermuda).

Minor Consent: Parent/Guardian of minors aged 10-17: Do you consent to your child being tested for COVID-19 while in Bermuda? If no, your child will have to quarantine for 14 days at your accommodation.
